Family Support Worker

The Christian County Health Department is accepting applications for a full-time Family Support Worker/Home Visitor. This position serves under general direction of the HANDS Supervisor.  Responsibilities include but are not limited to: Conduct regular planned visits to establish a trusting rapport for voluntary eligible HANDS participants, assist qualifying parents and families with child development, parenting skills, health services and other needed resources identified at assessment or during visits; Maintain confidentiality of program information obtained from home visits and exchanged while communicating with community and support service providers; Initiate and maintain regular and long term contact and provide support during the prenatal period and after according to program guidelines; Home visitations that may take place in the client’s home or another community site if justified, that include (a) monitoring of the child’s, mother’s and family’s progress by making referrals to community resources; such as prenatal health services, school, head start, etc, tracking appointments to ensure they are being kept, performing follow-up services as identified and performing periodic evaluations of the participants’ changing needs, (b) preparation and maintenance of case records which shall be documented with contacts, services needed, reports and progress, (c) consultations and (d) crisis assistance, confer with Supervisor in cases involving potential collaboration with other service providers.

 

Minimum Education, Training or Experience:  High School Diploma or GED and experience in a Social or other health related field such as but not limited to, working with children, teens and adults in a daycare, medical office or facility, school etc., that provides a medical or a community support service. 

 

Special Requirements:  Must be 18 years of age

Must have telephone accessibility

Must have reliable transportation and valid Drivers License

Criminal Background Check

CPR Certification

Working knowledge of geographic area and/or ability to locate participants and community resources with use of maps, contacts and directions provided.

HANDS Family Support Worker training.

 

Compensation:  $8.10 – $9.88 per hour, negotiable within this range based on experience and education above requirements. Grade 8

Local Health Nurse - Part Time

The Christian County Health Department is accepting applications for a Part Time Local Health Nurse I at our Oak Grove WIC Clinic.

 

General Duties include:  This position serves under the direction of the Nurse Supervisor.  Responsibilities for this position include, but are not limited to; Provide basic entry level nursing care and assessments to the client consistent with the Public Health Practice Reference, Nursing Scope and Standards of Practice.  Establish policies, procedures and licensing laws.  Document services provided to the client according to standards of the Core Clinical Service Guide (CCSG) and Administrative Reference (AR).  Maintain confidentiality and privacy according to guidelines.  Develop and conduct educational counseling and teaching activities for individual clients and families.  Utilize basic awareness of community resources and complete required trainings.

 

Minimum Education, Training or Experience:  See special requirements.

 

Special Requirements:  (Age, Licensure, Regulation, Etc.)  Must have RN license in Kentucky or compact state or provisional license in Kentucky, meeting all requirements of KRS 314.041 and approval to practice as Registered Nurse Applicant (RNA).

 

Starting Salary:  $17.10-21.34/hr. negotiable with additional experience.  Grade 15

This position serves under the direction of the Nurse Manager.  Responsibilities include, but are not limited to: provide basic entry level nursing care and assessments to the client consistent with the Core Clinical Services Guide (CCSG), Nursing Standards of Practice and established policies, procedures and licensing laws; document services provided to the client according to standards of the CCSG and Administrative Reference (AR); maintain confidentiality and privacy according to HIPAA and agency guidelines; develop and conduct educational counseling and teaching activities for individual clients and families; utilize basic awareness of community resources and complete required trainings.
 
Minimum Education, Training or Experience:  Must have RN license in Kentucky or compact state or provisional license in Kentucky, meeting all requirements of KRS 314.041 and approval to practice as Registered Nurse Applicant (RNA).

Starting Salary:  $17.10 - $21.34/hr negotiable with additional experience.  Grade 15
